Carlon Publicity Authorization

Carlon is considering using information about the experiences the Customer named above (Customer) has had with any Carlon product for publicity purposes to be published by Carlon for distribution worldwide to Carlon customers, other interested persons, and/or disseminated to press and analysts. This information may be published and distributed in any number of forms, including but not limited to print media, electronic media, audio visual media or via the Internet.

Customer hereby authorizes Carlon to use the story submitted by Customer to Carlon for the purpose(s) described above. Customer agrees and acknowledges that Carlon will not be required to seek further approval from Customer in connection with its publication of such information for such purposes. Should Carlon wish to use the above information for any other purpose, Carlon agrees that it will provide the designated Customer representative with advance copies of any such information referencing Customer for approval prior to publication. Customer will be deemed to have approved any such information if it has not provided Carlon with comments within three (3) business days following submission to Customer of such advance copy. Consent to such publication and distribution will not be unreasonably withheld by Customer.

This Publicity Authorization is intended as the complete agreement on this subject matter between Carlon and Customer. This Publicity Authorization will not modify any obligations Carlon may have to Customer pursuant to any Nondisclosure Agreement or other agreement that may be in effect between the parties. This Publicity Agreement will be governed under the laws of the State of Ohio, excluding its conflict of laws rules. The parties have caused this Publicity Authorization to be executed by their duly authorized representatives on the date shown. Submission of this form constitutes Customer's agreement with all of its provisions. Again, thank you for taking the time to share your experiences.
